• Please be prepared to disclose your Covid-19 result.

• Do you have any of the following symptoms:

• Cough

• Shortness of breath

• Fever

• Body aches

• Diarrhea

• Loss of taste or smell

• Have you been in contact with anyone who tested positive for the Coronavirus?

• Have you been in contact with anyone who could have been exposed to the Coronavirus?
